چکیده

Abstract Funding Acknowledgements Type of funding sources: None. Background Left bundle branch area pacing (LBBAP) has emerged as a promising technique to deliver cardiac resynchronization therapy (CRT). Purpose We sought evaluate the feasibility single LBBAP lead connected dual-chamber ICD in patients indicated with CRT-D implantation. Methods The CROSS-LEFT study prospectively included consecutive sinus rhythm reduced ejection fraction and complete left block. A DF-1 was implanted at right ventricular apex, an through interventricular septum. Patients were defibrillator (ICD) atrial (RA port), (RV IS-1 defibrillation pin being capped. Atrioventricular delay optimized ensure fusion between native conduction from bundle. followed during 6 months. Results total 22 (median age 68 years, 77% male, 41% ischemic). resulted significant reduction median QRS width 161 128ms (p<0.0001). At months, sensing 18mV (range 5–31, IQR 9), threshold 0.5V 0.25–0.75, 0). Median LVEF significantly improved 27 45% months (p<0.0001), LV end-systolic volume 32% (p=0.0002), 67% considered super-responders. Conclusion In block biventricular defibrillator, CRT can be safely efficiently performed via ICD, is associated electromechanical reverse remodeling.

Left ventricular endocardial pacing improves resynchronization therapy in canine left bundle-branch hearts.

BACKGROUND We investigated the benefits of the more physiological activation achieved by left ventricular (LV) endocardial pacing (ENDO) as compared with conventional epicardial (EPI) LV pacing in cardiac resynchronization therapy. METHODS AND RESULTS In 8 anesthetized dogs with experimental left bundle-branch block, pacing leads were positioned in the right atrium, right ventricle, and at 8 ...

Cardiac Resynchronization Therapy Using a Dual Chamber Pacemaker in Patients with Severe Left Ventricular Dysfunction and a Left Bundle Branch Block

Through the use of a dual chamber (DDD) pacemaker, we achieved a cardiac resynchronization effect in a 51-year-old female patient who was transferred to our hospital from another hospital for an operation for three-vessel coronary artery disease. Her electrocardiogram showed a left bundle branch block (LBBB) and a prolonged QRS interval of 166 milliseconds. Severe left ventricle (LV) dysfunctio...

Cardiac pacing in left bundle branch/bifascicular block patients.

The primary concern in patients with bifascicular block is the increased risk of progression to complete heart block. Further, an additional first-degree A-V block in patients with bifascicular block or LBBB might increase the risk of block progression. Anesthesia, monitoring and surgical techniques can induce conduction defects and bradyarrhythmias in patients with pre-existing bundle branch b...

Reversible left bundle branch block should be mentioned in cardiac resynchronization therapy; A clinical case report

BACKGROUND Cardiac resynchronization therapy (CRT) is a medical device to help cardiac synchronized contractility by electrical impulses. Improvement of symptoms and left ventricular systolic function, reducing hospital admissions and mortality in patients with moderate to severe heart failure are the main benefits of administration of cardiac resynchronization therapy. CASE REPORT In this ar...

Left ventricular resynchronization therapy in a canine model of left bundle branch block.

Positive responses to left (LV) and biventricular (BV) stimulation observed in heart failure patients with left bundle branch block (LBBB) suggest a possible mechanism of LV resynchronization. An anesthetized canine LBBB model was developed using radio frequency ablation. Before and after ablation, LV pressure derivative over time (dP/dt) and aortic pulse pressure (PP) were assessed during norm...
